[Mason, Ohio, April 15, 2025] — OneCity for Recovery is honored to receive a $15,000 grant from the Jacob G. Schmidlapp Trust, Fifth Third Bank, N.A., Trustee, to support its HopeLine Care Navigation to Treatment program in Hamilton County. This funding will help expand access to free, client-centered substance use disorder (SUD) treatment navigation, ensuring that more individuals receive the critical support they need to enter recovery.
The HopeLine program operates across five counties in Southwest Ohio—Hamilton, Butler, Warren, Clinton, and Montgomery—providing boots-on-the-ground intervention for individuals struggling with SUD. With 10 dedicated Care Coordinators, the program focuses on two key areas:
Care Navigation to Treatment – Care Coordinators meet clients wherever they are, responding to overdoses, engaging with first responders, and conducting outreach through probation offices, jails, and community events. They connect individuals to treatment quickly, partnering with over 250 treatment providers to remove barriers to care.
Recovery Continuing Care – After treatment, clients receive ongoing support during their first 12 months of recovery, a period of heightened relapse risk. Coordinators proactively screen for warning signs and provide personalized recovery support to ensure long-term success.
